You should always check with your insurance provider to know the limits of your coverage. You should also be aware that not all insurance providers cover all well-child visits or vaccines. Call our office if you need to schedule a physical exam at any time. Vision screening may begin at 3 years of age, if children are cooperative, but is recommended at. If your child participates in sports, camps, or some other programs, a physical exam may be required within the past 12 months. Preventive visits should include a check of vision and hearing. If the baby is breastfed, this visit should be two to three days after bringing the baby home. *If your child’s age is not listed, no pre-visit forms are needed (besides your annual registration forms linked above)īeginning at 3 years of age, we recommend annual visits. The first well-child visit happens soon after the baby is born. Please complete the electronic required forms prior to your visit.
